WORK SCHEDULE
Monday-Friday, 8:30am to 5 pm
SUMMARY OF POSITION RESPONSIBILITIES
The Vocational Case Manager provides vocational and case management services to support clients throughout treatment and their transition into the community. The position assists clients with employment, education, benefits and identification, housing, community resources, and other needs that support successful program completion and community reintegration. The Vocational Case Manager works collaboratively with the clinical team, employers, and community agencies to address client needs, goals, and barriers.
ESSENTIAL DUTIES
1. Provide vocational and case management services based on individual client needs, goals, strengths, and barriers. 2. Conduct and document individual client sessions to address vocational goals, case management needs, resource coordination, and transition planning. 3. Facilitate and document vocational, work-readiness, life-skills, and other assigned groups and orientations. 4. Assist clients with resumes, job searches, applications, employment preparation, work schedules, and vocational planning. 5. Coordinate and monitor client work assignments, attendance, employment progress, work evaluations, and work-related concerns. 6. Assist clients with identification, benefits, education, housing, and other community resources. 7. Make referrals and coordinate services with employers, workforce and educational programs, government agencies, and community providers. 8. Support transition and discharge planning, including employment, income, housing, education, and other community needs. 9. Maintain timely and accurate client documentation, vocational records, referrals, employment information, and required tracking. 10. Communicate client progress, needs, and barriers with the Clinical Director, counselors, and treatment team and participate in assigned meetings. 11. Complete required employment-related documentation and coordinate information with Finance and other departments. 12. Perform other related vocational and case management duties as assigned. These duties consolidate the original responsibilities for orientations, individual vocational counseling, job assignments, attendance, resumes, work evaluations, job-search monitoring, worksite concerns, and clinical-team communication without repeating overlapping functions.

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS
Possess a Masters Degree in Vocational Rehabilitation Counseling from a recognized school or the equivalent with a concentration in vocational rehabilitation. Preferred at least two years of full time paid professional experience in vocational rehabilitation, at least introductory experience in substance abuse treatment, and at least one year of supervisory or management experience. Preferred two years continuous sobriety, if a recovering chemically dependent person. Prefer coordinator to possess strong leadership skills, thorough knowledge of vocational rehabilitation and mental health concepts, as well as being capable of organizing, managing, and continued development of the vocational program.
REQUIRED CERTIFICATION/LICENSE
Professional licensure/certification (Licensed Rehabilitation Counselor, Licensed Professional Counselor, Certified Rehabilitation Counselor, or National Certified Counselor) or the equivalent of three years mental health counseling with the ability to obtain licensure/certification. Current CPR/First Aid certification required.

May include lifting up to 25 pounds for files, computer printouts on occasion.
WORKING CONDITIONS
Primary environment: ambient room temperatures, lighting and traditional office equipment as found in a typical office environment.
