Job Description
About the Role NC Health Partners is seeking passionate and reliable Qualified Professionals (QPs) to join our Intensive In-Home (IIH) teams in Winston-Salem. The IIH QP provides structured, family-centered behavioral health services in the home and community to support youth with emotional or behavioral challenges. Working under the supervision of a Licensed Clinician, the QP partners with families, schools, and community systems to help youth build coping skills, strengthen relationships, and achieve stability. This contract position is ideal for professionals who want flexibility while making a meaningful impact in their community. Key Responsibilities Provide direct interventions and support to youth and families using strength-based and evidence-based approaches. Implement the Person-Centered Plan (PCP) in collaboration with the Licensed Clinician and family. Deliver a minimum of two clinical sessions per week per client (approximately two hours each). Provide services in the home, school, or community, depending on the youth's needs. Assist in crisis response, safety planning, and stabilization under clinical supervision. Participate in weekly team meetings and supervision sessions for staffing and skill development. Coordinate care with schools, DJJ, DSS, and other involved community partners. Empower families through practical skill-building in communication, structure, and problem-solving. Qualifications Bachelor's degree in a Human Services field (Psychology, Social Work, Counseling, Criminal Justice, etc.) and at least two years of full-time, post-bachelor's experience providing mental health services to youth/families; or Master's degree in a related field with one year of qualifying experience. Valid driver's license, reliable transportation, and ability to travel within a 50-mile radius of the assigned area. Must live within 30-45 minutes of the service region. Bilingual candidates strongly encouraged to apply. Preferred Experience Experience providing community-based or in-home therapeutic services. Familiarity with Medicaid documentation and service definitions. Understanding of family-systems, cognitive-behavioral, and trauma-informed practices. Experience collaborating with DJJ, DSS, and schools. Crisis-intervention and de-escalation experience.
