Position Overview The Population Health Manager (Registered Nurse) - Medicaid Managed Care oversees the day to day activities of the Population Health Program. This is accomplished through the direct supervision of case managers and care coordinators who provide care management, transition of care, care coordination, and other population management activities to the patients of CFHC, Inc. with an emphasis on patients with Medicaid. The manager is knowledgeable in the principles of case management, including Medicaid managed care and works closely with the CFHC, Inc.'s clinically integrated network. The manager provides case management services to a small case load of patients.
THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-ANNUAL BONUS, BI-WEEKLY PAY.
Essential Tasks Provides direct supervision to the case managers and care coordinators. This includes but is not limited to interviewing, hiring, providing training and orientation, distribution and determination of responsibilities, and the on-going evaluation of the staffs' performance. Monitors staff productivity and department success through regular reporting and communication to executive staff. Performs audits and run reports to monitor staff's adherence with CFHC, Inc., the Carolina Medical Home Network, and other regulatory agencies rules and regulations for providing and documenting case management services. Represents CFHC, Inc. at Carolina Medical Home Network meetings. Attend other meetings and participate in committees as assigned (i.e., Population Health CIT, etc.). Reviews software system functionality and develop and document efficient workflows for staff. Serves as point of contact for local hospitals' discharge planners to identify patients admitted to the hospital or patients who have used the emergency room in hopes of reducing 30-day hospital readmission rates or inappropriate emergency room use. Experience and Education Graduate of an accredited school of nursing. Licensed as a Registered Nurse in the State of North Carolina. Three years in a healthcare environment with supervisory/management background and experience in case management. Must have personal vehicle readily available to freely travel in the local area; between clinics, hospitals, health departments, etc. Schedule Monday to Friday 8am-5pm 8 hour shift Day shift Physical Requirements Constant standing and walking, occasional bending, stooping, lifting, reaching forward and overhead. May occasionally be required to lift patients, office equipment and paper, usually not exceeding fifty pounds of direct lift. Frequent computer work required.
