The primary purpose of your job position is to provide transportation in a safe and timely manner, in accordance with established policies and procedures, and as directed by your supervisor. ROLE and
RESPONSIBILITIES
Communicate with dispatcher on Two Way RadioBe able to operate a Hydraulic Lift.
Report to Dispatcher and pick up patient schedule daily.
Transport 5 - 6 patients per day to and from doctors' appointments.
Inspect van daily. (ex: check oil, tires, first aid kit & fire extinguisher)Ensure that all passengers are secured by seat belts.
Complete all necessary forms, (ex: DMA 408, van log, etc.)Collect co-payments and private payments from patientsOther related duties and responsibilities that may become necessary to assure that the needs of the facility are met.

EDUCATION/EXPERIENCE/QUALIFICATIONS
Be a minimum of 25 years of age and of good moral character.
