Job Description
Patient Transportation Driver (Part Time (up to 29 hours per week))
Location:
Jefferson County Position Overview The person in this position drives the
PHNCF/CF
patient transportation van. This person is responsible for picking up and returning patients to/from scheduled appointments. Communication with patients, Transportation Coordinator, Lead Transportation Driver regarding daily transportation activities.
Conduct regular inspection and utilize the van checklist for the purpose of ensuring the vehicles are up to our standards.
Will provide courier and mail services between all PHN and PHNCF locations. This includes, but is not limited to delivering mail and supplies, which can be medical, office and miscellaneous items. Also included is the pickup of mail, weekly comment cards and miscellaneous items.
Performs other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the Transportation Department. Compensation Position starts at $16.00 per hour, however, PHN recognizes experience, education and site location when determining wage. Location and Hours of Position Punxsutawney Community Health Center 200 Prushnok Drive, Punxsutawney Pa 15767 Additional Primary Health Network locations throughout the Jefferson County area Monday Friday 7:30am 4:00pm as needed

May be asked to work outside this schedule as needed based on schedule. This position is part-time. Job Qualifications/ Requirements Valid Driver's license
High School Graduate/GED or equivalent work experience
Ability to lift and carry 30 pounds
Good communication skills and interpersonal skills
Basic-Average Computer Skills
Ability to utilize GPS for driving locations
