Qualifications & Experience:
Current, unencumbered Physical Therapist license (or eligibility to obtain) Previous experience in hospital or SNF (Skilled Nursing Facility) settings preferred Strong assessment, evaluation, and therapeutic skills Ability to communicate clearly and compassionately with patients and team members Familiarity with regulatory documentation standards
Responsibilities:
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans Implement evidence-based therapeutic interventions for patients of varied backgrounds and conditions Coordinate with physicians, nurses, and therapy team to optimize patient outcomes Educate patients, families, and caregivers on therapy plans and progress Maintain meticulous records in accordance with facility and legal requirements
