Key Responsibilities:
Evaluate assigned patients and develop, implement, and modify patient-specific treatment plans in accordance with individual needs, goals, and physician directives Delegate and direct therapy services for assigned patients, ensuring effective collaboration across disciplines Design functional therapy programs to support safe, effective discharge and return to community activities Educate patients, families, students, peers, and self on therapeutic processes and objectives Complete timely and accurate documentation, including goal writing, assessments, and charge entry, in compliance with clinic and regulatory standards Promote patient and employee safety, adhering to clinical safety procedures for patient handling, infection control, and a safe working environment Participate in student training programs and actively contribute to performance improvement initiatives Maintain professional licensure and meet all continuing education requirements
Desired Skills & Experience:
Valid licensure as a Physical Therapist (PT) Experience in outpatient or hospital-based therapy settings preferred Strong clinical reasoning skills and a passion for evidence-based patient care Ability to educate patients and collaborate with multi-disciplinary teams Commitment to professional ethics, ongoing education, and organizational values
