Qualifications & Key Experience:

Active RN license (New Jersey or compact state) Minimum of one year of experience in Rehab/LTC or similar direct patient care roles Knowledge of safe patient and materials handling, infection control, and fall prevention Strong commitment to safety, quality, and collaborative care Competency in developing individualized plans of care and supporting families through rehabilitation Excellent communication, documentation, and organizational skills

Core Responsibilities:

Complete comprehensive nursing assessments within 8 hours of admission Create individualized care plans, initiate patient/family teaching, and engage in team conferences Maintain accurate, timely, and clear documentation Participate in rehabilitation programs, prioritizing education and safe patient handling Start discharge planning from day one and maintain open communication across care providers Supervise and delegate tasks to ancillary personnel, fostering a positive unit culture Implement and uphold hospital safety policies in every aspect of care Pursue ongoing professional development and contribute to team-building activities
