Post-Acute & Long-Term Care Position -

ARNP or PA-C Rejuvenation Medical Pay:

We are a small, independent, and growing medical group providing post-acute and long-term care services in skilled nursing facilities throughout the Puget Sound region. Our model emphasizes thoughtful, relationship-based care. We want our providers to spend time with patients, understand their goals and baseline, get to know the nursing and facility teams caring for them, and provide consistent follow-through rather than simply moving through a list of visits. We focus on quality care, strong relationships, provider autonomy, and supporting one another as a clinical team. Responsibilities Post-Acute and Skilled Nursing Care Provide comprehensive medical management for post-acute rehabilitation patients following hospitalization or acute illness. Assess and manage acute and chronic conditions, perform admission and hospital-return evaluations as appropriate, monitor rehabilitation and clinical progress, and address changes in condition as they arise. Long-Term Care Management Serve as a primary medical provider for long-term care residents, developing familiarity with each resident's history, baseline function, goals of care, chronic conditions, and ongoing clinical needs. Regulatory Visits Perform required and delegated regulatory visits consistent with CMS requirements, Washington law, facility requirements, and Rejuvenation Medical practice policy. Acute and Follow-Up Care Evaluate new complaints, changes in condition, medication concerns, abnormal laboratory or imaging findings, hospital returns, specialist recommendations, and other clinically appropriate follow-up needs. Interdisciplinary Collaboration Work closely with physicians, ARNPs/PAs, nursing, therapy, social work, pharmacy, hospice, specialists, and facility leadership. We consider nurses essential clinical partners. Providers are expected to listen to nursing concerns, communicate clinical reasoning and plans clearly, provide education when appropriate, and build collaborative relationships within each facility. Facility Integration Get to know the buildings, residents, and teams you work with. Participate in care conferences and relevant facility meetings when appropriate and maintain strong communication with facility leadership and clinical staff. Travel Between Skilled Nursing Facilities This is a facility-based position requiring travel between skilled nursing facilities throughout our service area. Providers will typically work at one facility for the day , rather than traveling between multiple facilities during the same workday. Facility assignments may vary based on scheduling, patient needs, provider coverage, and practice needs. Procedures Providers may perform procedures when clinically appropriate and within their individual scope of practice, training, privileges, and the capabilities of the facility. Training and support can be provided for appropriate procedures when needed. On-Call Rotation Participation in the shared provider on-call rotation is required, phone only. Charting in Real Time We value your time. You'll get a scribe and access to advanced AI tools or software that improves your workflow. Your work device will be top-notch, with touchscreen laptops as standard, and requests for productivity-enhancing technology are rarely denied. Charts should be completed within 48 hours, with same-day as the goal. Providers remain responsible for reviewing documentation for accuracy and ensuring the final medical record accurately reflects the care provided. Other Duties Include Tracking clinically relevant patient-care information and outcomes Participating in care planning and interdisciplinary facility discussions Medication management and reconciliation Reviewing laboratory, imaging, pharmacy, therapy, and consultant recommendations Coordinating transitions of care and hospital returns Participating in goals-of-care and advance-care-planning discussions when appropriate Collaborating with hospice and palliative-care teams when appropriate Mentoring or precepting when desired Maintaining compliance with applicable SNF/LTC clinical and regulatory requirements Contributing to a positive, collaborative, team-focused culture What We're Looking For Current Washington State ARNP license in good standing or PA-C licensure/credentialing appropriate for practice in Washington Current national board certification appropriate to professional role Prescriptive authority appropriate to the position DEA registration or eligibility when required for assigned duties Ability to successfully complete Medicare, Medicaid, payer, facility, and organizational credentialing requirements Experience in primary care, geriatrics, internal medicine, post-acute care, skilled nursing, or a similar clinical setting preferred Strong clinical judgment and comfort caring for medically complex adult and geriatric patients Skilled in communication and interdisciplinary collaboration Self-directed with strong organizational habits Comfortable working independently while knowing when to seek consultation Comfortable working in skilled nursing facilities or motivated to learn this practice environment Respectful and collaborative approach to nursing and facility staff Appreciation for the nuance, unpredictability, and complexity of nursing-home medicine A good sense of humor and a desire to grow with a passionate team Job Types Full-time, Part-time, Contract (1099) Full-Time Benefits Include Dental insurance Flexible schedule Health insurance Paid time off Vision insurance Part-time and contract arrangements, compensation structure, and benefits eligibility will depend on the specific position and agreement.
