Additional weekday shifts may be available based on employee availability Position Summary The Administrative Coordinator provides clerical and administrative support to the Case Management team. This position assists with discharge planning, insurance verification, financial assessments, referrals, and coordination of inpatient and post-hospitalization care.
Important:
The selected candidate must be available Monday-Friday for orientation and training until training is completed. Key Responsibilities Discharge Planning Support Case Managers with discharge planning and follow-up activities.
Coordinate referrals to outside agencies and follow up on required paperwork.
Assist with arranging durable medical equipment (DME), medications, and transportation.
Perform clerical duties including faxing, copying, telephone calls, and documentation.
Ensure discharge plans are followed through to completion. Financial & Insurance Coordination Verify insurance coverage and obtain benefits information.
Assist with financial resource assessments and refer patients to financial counselors when needed.
Maintain knowledge of common reimbursement methods and payer requirements.
Assist with continued-stay documentation, denial information, disability applications, and county transfers.
Support Medi-Cal reviews, TAR completion, insurance denials, and appeals.
Follow up on pre-certification activities and admission notifications. Clinical & Case Management Support Participate in daily Case Management team meetings.
Assist with admissions, transfers, and discharge documentation.
Help ensure required test results and records are available for physician and Case Management review.
Monitor frequent admissions and notify Case Managers as appropriate.
Assist with obtaining resources and completing required case management paperwork. Psychosocial & Patient Support Screen cases for advance directive documentation.
Assist patients and families with required paperwork and healthcare decision-making processes under the direction of the RN/Social Work Case Manager.
Complete mandated legal reporting as appropriate.
Communicate relevant patient and payer information to the Case Management team. Team Support Coordinate with RN Case Managers, Social Workers, Admitting, and insurance companies.
Facilitate timely transmission of utilization review documentation.
Maintain awareness of managed care contract requirements and payer regulations.
Collect and enter data related to avoidable delays and patient days.
Maintain strict confidentiality of protected health information in accordance with HIPAA and hospital policies.
Follow all patient safety and departmental policies. Required Qualifications High School Diploma or equivalent.
Previous experience working in a medical or healthcare setting.
Basic computer skills.
Knowledge of medical terminology.
Strong verbal and written communication skills.
Strong critical-thinking and problem-solving abilities.
Must be able to work onsite.
Must be available Monday-Friday during the orientation/training period.
LA City Fire Card:
Required within 6 months of employment for PHGSH. Preferred Qualifications Bachelor's degree preferred.
Previous experience supporting Case Management, discharge planning, utilization review, insurance verification, or healthcare administration is a plus. Ideal Candidate The ideal candidate is organized, detail-oriented, and comfortable working in a hospital environment while coordinating multiple administrative tasks. Experience with patient discharge processes, insurance/payer coordination, and Case Management support is highly preferred.
