Placement Manager Part-Time | Solano County, CA About the Opportunity Hazel's Tranquility Place, in partnership with BrandGOV, Match2Hire, and Solano Impact Care, are seeking an experienced, organized, and responsive Community & Referral Relations Coordinator to oversee referrals from initial review through approved placement and move-in. The role works closely with prospective clients, referral partners, health plans, house managers, medical support staff, ECM staff, and program leadership. The ideal candidate already understands CalAIM and has hands-on experience with Partnership HealthPlan and/or ILS/Kaiser workflows. This person should be able to identify missing information, confirm the appropriate service and next step, and keep multiple referrals moving without losing follow-up. They should also be a natural relationship-builder who is continuously strengthening community referral pathways, developing a qualified placement pipeline, and using referral and bed-utilization data to identify opportunities.

WHAT YOU WILL DO
Referral Review & Eligibility Review incoming referrals and contact the prospective client or referral source to confirm eligibility, requested services, immediate needs, discharge timing, and missing information. Verify payer/health plan, including Partnership HealthPlan and ILS/Kaiser, and confirm the requested CalAIM service or program. Review medical, functional, behavioral, housing, and safety information; identify issues that require clarification or supervisory review. Confirm client/facility compatibility and current bed availability before moving a referral forward. Placement & Authorization Coordination Coordinate complex or urgent referrals with program leadership, house managers, medical support staff, ECM staff, and other designated reviewers. Review authorization or TAR status, requested service dates, service codes/units, and supporting documents when applicable. Prepare a placement recommendation and obtain required approval before communicating acceptance or a move-in date. Document referrals that are pending, declined, canceled, redirected, or placed on a waitlist. Move-In & Tracking Coordinate approved move-in dates, transportation, room readiness, intake appointments, required forms, and staff handoff. Maintain the referral/placement tracker with status, missing items, eligibility review, authorization status, decision date, assigned site, move-in date, and closure reason. Keep active-client and service-assignment information current so staff know which clients and services they are responsible for. Monitor early placement concerns and communicate significant issues promptly to program leadership and the assigned house/care team. Community Partnerships & Placement Pipeline Build and maintain strong working relationships with hospitals, health plans, county agencies, community-based organizations, housing partners, and other referral sources throughout Solano County. Proactively identify organizations and partners that can strengthen referral pathways, support appropriate bed placements, and improve coordination for clients. Develop and maintain a qualified placement pipeline and waitlist so available beds can be filled efficiently with appropriate clients. Regularly review referral, placement, bed-availability, and business-tracking data to identify gaps, trends, stalled referrals, and opportunities for stronger community partnerships. Take ownership of the placement pipeline—ensuring prospective placements are actively followed, relationships are maintained, and opportunities do not fall through the cracks.
WHAT YOU BRING
Required Experience Professional experience with CalAIM, healthcare referrals, care coordination, admissions, placement, or managed care. Experience with Partnership HealthPlan and/or ILS/Kaiser referral, authorization, or placement workflows. Working knowledge of ECM, Community Supports, Medical Respite/Recuperative Care, or related CalAIM services. Strong attention to eligibility, dates, codes, authorizations, required documents, and follow-up. Professional communication with clients, hospitals, health plans, county agencies, and community partners. Strongly Preferred Direct experience with both Partnership HealthPlan and ILS/Kaiser. Experience with TARs, payer portals, authorization tracking, or hospital discharge referrals. Experience with homelessness, medical recovery, behavioral health, substance-use recovery, or justice-involved populations. Established professional relationships within Solano County's healthcare, housing, behavioral health, homelessness-response, or community-services network. Strong analytical and organizational mindset; enjoys using referral and placement data to identify opportunities, prioritize follow-up, and improve bed utilization. Experience with SimplePractice or another EHR, Google Sheets, referral trackers, and electronic workflow systems. Ability to manage multiple active referrals and deadlines with minimal supervision.
