Advanced Practice Practitioner (Physician Assistant) 3.6 3.6 out of 5 stars Indio, CA From $150,000 a year - Full-time Desert Oasis Healthcare 76 reviews From $150,000 a year - Full-time The Advanced Practice Practitioner (APP) is a licensed Provider who works under the direction of the Medical Director to assess patient's health, provide medical treatment and create treatment/care plans for patients. The Advanced Practice Practitioner refers patients for other healthcare services as deemed appropriate to support the continuity of care and ensure that the patient receives holistic and comprehensive care. Professional Duties Obtain patient medical history and conduct a thorough assessment of each patient's health status. Order diagnostic tests and referrals for other healthcare services as deemed appropriate to support the continuity of care and ensure that the patient receives holistic and comprehensive care. Develop an effective treatment plan and establish a diagnosis for each patient. Revise patient treatment plans based on the progression and ongoing status of patient's health. Communicate treatment plan and other care related topics to patients. Collaborate with case management team, as needed. Provide coverage for other Providers as requested. Document all patient interactions, visits and services rendered in patient's electronic health record (EHR) accurately and timely in accordance with departmental and organizational standards. Understand the importance of HCC coding requirements and 5-star standards. Attend and participate in meetings, as requested. Submit time off requests in accordance with organizational and departmental policies and procedures. Perform other duties as assigned. Be an active listener (make eye contact, validate) Assume a neutral position and suspend judgment of others Speak clearly and use understandable language (avoid medical jargon or slang) Speak positively about your work, co-workers and Desert Oasis Healthcare and Family Hospice Care Qualifications Master of Science in Nursing (MSN), Doctorate of Nursing Practice (DNP), or Completion of an Accredited Physician Assistant (PA) program. Current unrestricted license to practice medicine in California. Current Drug Enforcement Agency (DEA) license. 1 year of comprehensive healthcare experience. Ability to effectively communicate with individuals within all levels of the organization, providers, patients, patient family members, vendors and others. Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications. Ability to manage time effectively and prioritize tasks to meet established deadlines. Ability to utilize critical thinking and personal judgement to problem solve. Current California driver's license and proof of auto insurance coverage.
