Job Description

If so, we have an exciting opportunity for you to work as a Supervisor on our Community Health Worker Outreach Team! What You Will Be Doing Working as a Program Supervisor you will be responsible for managing the team of community health workers (CHWs) who work with individuals experiencing barriers to their social determinants of health (SDoH) needs. CHWs provide direct assistance, as well as outreach in the community, to at risk populations with identifying SDoH resources and the navigation of those resources. In this supervisory role you will have the opportunity to not only manage the CHW team but assist in enhancing this program through tracking and analyzing data, building and strengthening partnerships with community providers and resources, leading team and agency-wide meetings regarding the operations of the program, and remaining well-informed of developments and changes to the CHW certification process and programming within the state of Michigan. In this role you will demonstrate passion and cultural competence when working with staff to have successful engagement with individuals needing these supports. This is a hybrid position where you will split your work time between the office, community, and home. What We Require A High School Diploma or GED with a minimum of three years of work experience in mental health, healthcare, or community resources. Must be a Certified Community Health Worker with at least one year directly assisting those diagnosed with a mental illness or co-occurring disorder with Social Determinants of Health. Experience in team supervision with demonstrated leadership skills and an ability to train/mentor other team members is preferred. A valid Michigan Driver's License is required. Physical Requirements/Working Conditions:
Physical Efforts - Job demands include prolonged sitting and standing as appropriate. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eye/hand/foot. Working Conditions - Office environment with noise from computers, copy machine, and telephones. Use of video display terminal (VDT) for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of viewing VDT. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
