3 days/week initially | Opportunity to expand to 5 days/week We are a growing, independent healthcare practice in Chandler, Arizona seeking a compassionate and motivated Family Nurse Practitioner (FNP) to join our team and help grow our primary care services. This is an excellent opportunity for an FNP looking for a part-time position with the potential to grow into a larger role . The position will initially be 3 days per week , with the opportunity to expand to 5 days per week as the patient panel grows . What You'll Do Provide comprehensive primary care to adult and pediatric patients Conduct thorough patient assessments, including physical examinations, and medical history reviews using electronic health records (EHR) systems Perform annual physicals and Annual Wellness Visits Manage acute and chronic medical conditions Prescribe medications and treatment plans in accordance with state regulations and collaborative practice agreements, ensuring compliance with DEA regulations. Order and interpret laboratory and diagnostic testing, and other advanced tools to inform clinical decision-making. Provide preventive care and patient education Manage medications and prescription refills Coordinate referrals and specialty care Provide patient education on disease management, medication administration, nutrition, and health coaching to promote wellness and preventive care. Engage in telehealth services to extend access to care for remote or underserved populations while maintaining HIPAA compliance. Maintain timely, accurate clinical documentation of all patient interactions within EMR/EHR systems for efficient medical records management. Collaborate with the practice owner and clinical team to provide high-quality, patient-centered care What We're Looking For Board-certified Family Nurse Practitioner (FNP-C or FNP-BC) Active or eligible Arizona APRN license DEA registration or ability to obtain Primary care experience of 1 year preferred Comfortable practicing with a high degree of independence Strong clinical judgment and communication skills Reliable, organized, and attentive to detail Warm and professional bedside manner Interested in developing long-term relationships with patients Motivated to grow with an independent practice.
