The ideal candidate will provide comprehensive, patient-centered care to adult patients across a broad range of acute and chronic medical conditions. This role requires strong clinical judgment, excellent communication skills, and the ability to work collaboratively with physicians, specialists, nursing staff, and other members of the healthcare team. The APNP will be responsible for evaluating patients, establishing diagnoses, developing individualized treatment plans, managing chronic diseases, addressing acute medical concerns, and promoting preventive health. The successful candidate will play an integral role in improving patient outcomes, enhancing access to care, and ensuring continuity and coordination throughout the patient's healthcare journey. Responsibilities Conduct comprehensive patient evaluations, including detailed medical histories, physical examinations, medication reconciliation, and review of pertinent diagnostic and laboratory results. Assess, diagnose, and manage a broad range of acute and chronic medical conditions commonly encountered in Internal Medicine. Develop, implement, and monitor individualized treatment plans based on current evidence-based guidelines, patient needs, clinical findings, and risk factors. Order, interpret, and follow up on appropriate laboratory studies, diagnostic imaging, and other medical tests. Prescribe medications and therapies within the scope of practice and applicable state and federal regulations. Manage patients with complex and chronic conditions, including hypertension, diabetes, hyperlipidemia, cardiovascular disease, respiratory disorders, gastrointestinal conditions, and other common medical problems. Provide preventive healthcare services, including health screenings, immunizations, wellness counseling, and risk-factor assessment. Monitor treatment effectiveness and modify care plans as clinically indicated. Identify changes in a patient's condition and determine when escalation of care, emergency intervention, or referral to a physician or specialist is warranted. Collaborate closely with physicians, specialists, nurses, pharmacists, therapists, and other healthcare professionals to coordinate care and optimize clinical outcomes. Facilitate appropriate referrals and ensure timely communication and follow-up between the patient, primary care team, and specialists. Educate patients and their families regarding diagnoses, treatment options, medications, disease management, lifestyle modifications, and preventive healthcare. Promote patient engagement and shared decision-making by ensuring patients understand their medical conditions and available treatment options. Perform minor office-based procedures and interventions within the APNP's training, competency, and authorized scope of practice. Maintain complete, accurate, and timely electronic medical records, including assessments, diagnoses, treatment plans, prescriptions, patient education, and follow-up instructions. Ensure all clinical documentation complies with applicable regulatory requirements, professional standards, and organizational policies. Participate in quality-improvement initiatives designed to improve patient safety, clinical outcomes, efficiency, and patient satisfaction. Remain current with advances in Internal Medicine, clinical guidelines, pharmacologic therapies, and evidence-based treatment approaches. Maintain professional licensure, certification, continuing education, and clinical competencies as required. Provide compassionate, culturally responsive, and respectful care while maintaining patient confidentiality and professional standards at all times. Qualifications Current and unrestricted APNP/Advanced Practice Registered Nurse (APRN) license in the applicable state. National board certification as a Nurse Practitioner. Prescriptive authority and appropriate state-required credentials. Current BLS certification; additional certifications may be required based on clinical responsibilities. Strong clinical assessment, diagnostic reasoning, and decision-making skills. Excellent interpersonal, communication, organizational, and documentation skills. Ability to work independently while functioning effectively as part of a multidisciplinary healthcare team.
