Per Diem Mobile Wound Care Nurse Practitioner We are seeking an experienced Nurse Practitioner (NP) to provide mobile wound care services on a per diem basis . This role is intended for a provider who already has hands-on wound care experience and is comfortable independently performing treatments ordered or approved by the supervising physician. The NP will travel to patients in their homes, assisted living facilities, skilled nursing facilities, or other approved care settings. Scheduling will vary based on patient availability, family coordination, geographic location, and travel requirements. All wound care supplies, dressings, and treatment materials required for assigned visits will be provided by our organization. Responsibilities Travel to scheduled patient locations to provide mobile wound care services. Perform focused wound assessments, including wound measurements, tissue characteristics, drainage, surrounding skin condition, and healing progress. Carry out wound care treatment plans prescribed or approved by the supervising physician. Perform conservative wound care, including: Wound cleansing Dressing changes Compression or offloading support as ordered Selective or excisional debridement when clinically appropriate and within scope of practice Apply advanced wound care products when ordered and medically necessary, including: Collagen-based wound products PuraPly AM or similar advanced wound matrices Amniotic or other cellular/tissue-based skin substitute products Take wound photographs and measurements when required for documentation. Document all treatments, wound progress, products used, patient response, and follow-up needs accurately and promptly. Identify and report signs of infection, wound deterioration, or other clinical concerns to the supervising physician. Maintain proper sterile or aseptic technique and follow infection-control standards. Educate patients, caregivers, and family members regarding wound care, dressing management, pressure relief, nutrition, and prevention. Communicate effectively with the supervising physician, patient families, facility staff, and other members of the care team. Follow HIPAA requirements, company clinical policies, and applicable payer and regulatory requirements. Supplies and Equipment Our organization provides the necessary wound care supplies and treatment materials for scheduled visits, including appropriate dressings, collagen products, advanced wound care materials, and other physician-ordered supplies. The NP is responsible for maintaining, transporting, and using supplied materials appropriately during assigned visits. Position Structure This is a per diem mobile position , and the schedule may change from week to week.

Patient and family availability Geographic location Travel distance Clinical need Treatment frequency Facility access Provider availability Candidates should be comfortable with a flexible schedule and traveling between patient locations. Qualifications Current and unrestricted Nurse Practitioner license in the state where services are provided. Minimum 1 year of hands-on wound care experience required. Experience managing chronic and complex wounds. Experience performing wound debridement. Experience with advanced wound care products, collagen products, skin substitutes, or cellular/tissue-based products strongly preferred.
Familiarity with wounds such as:
Pressure injuries Diabetic ulcers Venous ulcers Arterial wounds Surgical wounds Chronic non-healing wounds Comfortable working independently in home, assisted living, skilled nursing, and mobile-care environments. Strong wound documentation and photography skills. Reliable transportation and willingness to travel. Strong communication skills and professionalism when working with patients, families, physicians, and facility staff. Current BLS certification preferred. Ideal Candidate We are specifically looking for an experienced wound care provider who can step into the role with minimal training . The ideal candidate understands wound staging, debridement, advanced dressings, collagen products, skin substitutes, documentation requirements, infection prevention, and when changes in a wound should be escalated to the supervising physician. This is an excellent opportunity for an NP seeking flexible per diem work while providing hands-on wound care in a mobile setting with physician oversight and company-provided wound care supplies.
Pay:
$65.00 - $110.00 per hour Expected hours: 10.0 per week
