We are looking for an experienced Home Health RN Supervisor to provide clinical leadership, supervision, and oversight while supporting the quality and compliance of our home health services. Position Summary The Home Health RN Supervisor provides clinical supervision, guidance, and oversight to nursing personnel and assists with the overall supervision and direction of healthcare services. This position plays an important role in ensuring the quality and appropriateness of patient care , supporting staff development and performance, maintaining compliance with applicable State and Federal guidelines, and helping ensure clinical services are delivered according to patient needs and the physician's Plan of Care. The RN Supervisor will work closely with clinical staff and leadership to provide guidance, address concerns, support quality improvement, and promote excellence in patient care. Responsibilities Provide clinical supervision, guidance, and oversight to nursing personnel. Assist with the overall supervision and direction of healthcare services. Ensure clinical services are provided in accordance with applicable State and Federal guidelines and nursing standards . Conduct and support clinical audits and quality reviews. Provide training and education to clinicians through virtual and field-based training . Review the quality and appropriateness of patient care and clinical practices. Review and revise nursing care plans and the physician's Plan of Care as needed. Maintain regular communication with nursing staff and case managers regarding patient care and clinical needs. Evaluate staff performance and provide ongoing coaching, mentoring, and clinical guidance . Address patient and family complaints and assist with appropriate resolution and action plans. Assign personnel to patients based on patient needs and the physician's Plan of Care. Coordinate skilled services and personal caregivers based on patient needs. Evaluate patient eligibility and appropriate level of care. Oversee initial and ongoing in-service education and training . Assist with recruiting, interviewing, hiring, and orientation of clinical staff. Support staff development and promote professional growth. Participate in performance evaluations, counseling, and disciplinary actions when appropriate. Collaborate with Billing and Payroll as needed to support clinical operations. Maintain confidentiality of patient and agency information. Support continuous quality improvement and compliance initiatives. QualificationsRequired Minimum of three (3) years of nursing experience within the past five (5) years in a home health agency, primary care clinic, or health facility 3 years of supervisory experience Ability to be on the premises or immediately accessible by telecommunications during operating hours when patients are receiving services, consistent with California requirements Demonstrated ability to provide clinical supervision, guidance, and oversight to nursing personnel Knowledge of home health clinical practices, patient care planning, documentation, infection prevention, and applicable standards of care Tech savvy - ability to work with several digital products such as EMR, Google Suite, etc. Proficient with Microsoft Office products (Outlook, Word, Excel, PowerPoint) Able to travel to prospective patient's place of residence as needed (part-time) Desired Ability to demonstrate strong communication, presentation, interpersonal, analytical, and problem-solving skills Leadership experience Previous Assistant DON, Clinical Manager, Clinical Supervisor, or Director of Nursing experience Experience with OASIS documentation and Medicare regulations Experience leading quality improvement initiatives Strong coaching and mentoring abilities Required Credentials California RN License Driver's License Auto Insurance CPR/BLS Certification TB Test or Chest X-Ray Physical Examination Professional Liability SSN Physical Requirements Ability to travel to prospective patients' residences as needed. Ability to work in healthcare and home-based settings. Ability to effectively communicate with patients, families, clinicians, and healthcare professionals.
