Occupational Therapist (OT) - Home Health PRN At Home Healthcare | Greater New Orleans Area At Home Healthcare is seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team serving the Greater New Orleans area . We are looking for an Occupational Therapist who enjoys working independently, building meaningful relationships with patients, and helping individuals regain the skills and confidence they need to remain safe and independent in their own homes. What You'll Do Perform comprehensive occupational therapy evaluations in patients' homes Develop individualized plans of care based on each patient's functional abilities, needs, and goals Provide skilled occupational therapy interventions to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Address functional limitations involving strength, coordination, balance, cognition, safety, and mobility as appropriate Assess the home environment and recommend modifications to improve patient safety and independence Evaluate patients for appropriate adaptive equipment and durable medical equipment Provide patient and caregiver education, including techniques to promote safe and effective completion of daily activities Assess patient progress and modify treatment plans as clinically appropriate Collaborate with physicians, nurses, physical therapists, speech therapists, and other members of the interdisciplinary care team Complete accurate and timely clinical documentation in accordance with agency, Medicare, and regulatory requirements Communicate changes in patient condition and participate in ongoing care coordination Qualifications Current, unrestricted Louisiana Occupational Therapist license Graduate of an accredited Occupational Therapy program Current CPR/BLS certification Valid driver's license, reliable transportation, and automobile insurance Strong clinical assessment, communication, and organizational skills Ability to work independently while remaining an active part of an interdisciplinary team Home health experience is preferred, but not required Why Join At Home Healthcare?
