Home Health Physical Therapist Assistant

Job Description

CORA Physical Therapy seeks a Physical Therapist - Digital Health Specialist to deliver evidence-based outpatient rehab through a blend of in-clinic and virtual care. In this role, you'll evaluate and treat patients with orthopedic and neurologic conditions, design personalized plans of care, and leverage telehealth platforms and digital tools to enhance access, engagement, and outcomes. You'll document efficiently in our EHR, track objective progress, and collaborate closely with a multidisciplinary team in a supportive, patient-first culture focused on clinical excellence, respect, and professional growth. CORA offers robust continuing education, mentorship, and opportunities to innovate in digital rehab while helping patients regain mobility, independence, and quality of life. Responsibilities Provide evidence-based physical therapy in-clinic and through secure digital/telehealth platforms Evaluate patients, develop individualized plans of care, and update treatments based on progress Use digital health tools and EHR systems to document, track outcomes, and optimize visit utilization Educate patients and families on home exercise programs, prevention, and long-term self-management Collaborate with physicians, therapists, and clinical staff to coordinate comprehensive care Ensure high standards of clinical quality, patient safety, and regulatory compliance Participate in continuing education, mentorship, and clinical in-services to advance skills Contribute to a patient-first, team-based culture aligned with CORA's values Support scheduling efficiency and caseload management across in-person and virtual visits Engage in quality improvement and digital health innovation initiatives Required Skills Orthopedic physical therapy Neuromuscular rehabilitation Telehealth service delivery Digital health documentation/EHRTreatment plan development Outcome measurement and data tracking Patient education and engagement Pain management techniques Post-surgical rehab protocols Interdisciplinary team collaboration
