Schedule:

Monday through Friday, 8:00 AM 5:00

PM Key Responsibilities:

Conduct OASIS assessments to develop tailored care plans Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management Monitor patient conditions and communicate updates to the broader care team and physicians Educate patients and families about diagnoses, treatments, and preventive health strategies Ensure thorough documentation using Home Care Home Base (HCHB) Work collaboratively with physical and occupational therapists, medical social workers, and home health aides Attend case conferences, staff meetings, and ongoing training as required Uphold all regulatory and professional standards for patient care and documentation Travel daily to patient homes within your assigned territory (mileage reimbursement offered)

Required Qualifications:

Active and unrestricted RN license for Illinois At least 1 year of recent home health nursing experience (field experience desired) Advanced proficiency in Home Care Home Base (HCHB) and experience with OASIS Valid drivers license, reliable transportation, and proof of auto insurance
