Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage plan members in their residences Review and document prior/current medical history, ongoing health conditions, and medication regimens Educate members about their diagnoses, treatment, and proper medication use Identify care gaps and recommend appropriate measures for improved outcomes No need to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner licensure in North Carolina Board Certification through AANP or ANCC Current CPR (BLS) certification Graduation from an accredited MSN program At least 6 months of recent Nurse Practitioner experience (preferred)
