Responsibilities Conduct thorough patient assessments to evaluate psychological, social, and emotional needs using clinical interview techniques and diagnostic tools. Develop and implement tailored care plans that align with clinical ethical guidelines and social work practice standards. Coordinate community resources and support services to assist patients with housing, financial assistance, transportation, and other social determinants of health. Document all interactions accurately while maintaining compliance with HIPAA regulations. Collaborate closely with interdisciplinary teams—including medical professionals, nurses, therapists, and case managers—to ensure holistic patient care. Requirements Valid state licensure as a Certified Social Worker (CSW). Proven experience in social work practice within healthcare settings. Strong knowledge of medical terminology, ICD-10 coding standards, and Medicare/Medicaid policies. Demonstrated expertise in counseling, behavioral health interventions, and working with diverse cultural backgrounds. Ability to perform patient assessments accurately and develop appropriate treatment plans aligned with clinical ethical guidelines. Excellent communication skills for client interaction and interprofessional collaboration. Certifications in CPR/BLS (Basic Life Support) are preferred. Experience working with geriatric patients, individuals with developmental disabilities or autism spectrum disorder (ASD), dementia care or Alzheimer's care is highly desirable.
