Principal Job Functions:

Client Services Engages in and develops trusting relationships with clients Respect the range of diverse cultures and the broad array of needs experienced by individuals and families. Assists the family in developing an individualized support network. Provides services to clients in their home or community setting. Maintains active and regular contact with clients, including at least one face-to-face contact per month. Engages clients in developing their goals. Provides parent educational/skill building and/or emotional support to families. Supports clients with information, tools, and resources they need to develop new competencies and become their own advocates. Works cooperatively with program partners to secure housing and related support services for clients. Connects clients to needed mainstream and community resources. Provides client transportation, using personal vehicle. Provides regular updates to referral sources or other involved parties. Participates actively in supervision to better meet the needs of clients.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

Knowledge of community resources and case management best practices, including Motivational Interviewing and evidenced-based housing models and related support services. Ability to work with a variety of individuals from diverse backgrounds. Ability to work cooperatively and professionally with community partners and representatives from other agencies. Ability to work independently yet utilize supervision as needed. Ability to drive and provide proof of driver's license and auto insurance.

EDUCATION AND EXPERIENCE

Bachelor's Degree in a related field. Minimum of twelve months of previous experience working with individuals and families from diverse backgrounds.
