Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach and housing program. As a Service Coordinator/Re-Entry Specialist with Chartiers Center , you will play a vital role in supporting individuals experiencing homelessness who are living with mental health challenges and have current or prior involvement with the justice system. This position focuses on helping participants achieve housing stability, access essential community resources, and successfully reintegrate into the community.

Job Duties Include :

Collaborate with inpatient psychiatric units, emergency departments, correctional facilities, probation and parole offices, and other justice system partners to ensure effective care coordination and seamless transitions for individuals returning to the community. Serve as a liaison and support resource, helping individuals navigate complex systems while coordinating services that promote recovery, wellness, housing stability, and successful community reintegration. Assist individuals and families in identifying, obtaining, and maintaining permanent housing by addressing barriers, connecting them to community resources, and advocating for their needs. Conduct outreach and provide direct services in a variety of community-based and non-traditional settings, including individuals' homes, shelters, streets, healthcare facilities, correctional facilities, and other locations where services are needed. Develop and implement individualized service plans that support participants' mental health, physical health, housing, and recovery goals. Facilitate referrals and linkages to behavioral health services, medical care, substance use treatment, public benefits, employment resources, and other supportive services. Monitor participant progress, regularly assess ongoing needs, and coordinate appropriate interventions to promote long-term housing retention, recovery, and overall well-being. Establish and maintain collaborative relationships with community providers, landlords, healthcare professionals, and criminal justice stakeholders to improve access to services and support positive outcomes. Provide advocacy, crisis intervention, and problem-solving assistance to help individuals overcome barriers that may impact housing, treatment engagement, or community stability. Maintain accurate documentation, service records, and reporting requirements in accordance with agency, funding source, and regulatory standards. Utilize a person-centered, trauma-informed approach in all interactions to empower individuals and families to achieve greater independence, self-sufficiency, and quality of life. Friday Qualifications:

Bachelor's degree in Human Services or related field with at least one year experience in justice related services. Associate's degree in Human Services or related field and two years experience.
