Financial Representative II - IntermediateCommunity Resource Coordinator Pennsylvania Department of Human Services - Bureau of Human Services Licensing Help connect licensed personal care homes and low-income residents with critical community resources and services. We are seeking a Community Resource Coordinator (Financial Representative II - Intermediate) to build partnerships with community organizations, assess facility needs, coordinate resources, and provide technical assistance and training.

Compensation Options Tier 1 - Benefits-Eligible W-2: $26.00 per hour Tier 2 - Non-Benefits-Eligible W-2: $28.00 per hour 1099

Notice:

Independent contractors are responsible for their own taxes, insurance, and other applicable obligations. Key Responsibilities Serve as a liaison between licensed personal care homes and community organizations, including local agencies, professional boards, task forces, churches, nonprofit organizations, county programs, Area Agencies on Aging, and Ombudsman programs. Identify and coordinate resources for facilities serving low-income populations. Conduct on-site assessments to identify facility and resident needs and connect them with appropriate resources. Assist facilities with accessing available funding, food banks, eligible benefits, and other community-based services. Make and receive appropriate referrals and establish effective working relationships with community partners. Maintain a database of community resource providers and available services. Monitor and analyze the effectiveness of resource and service delivery. Troubleshoot emerging facility needs and help identify appropriate solutions. Participate in regional Risk Management meetings to identify concerns and advocate for additional resources. Provide presentations and training to licensed facilities, DHS staff, and community organizations. Coordinate with regional leadership to identify facilities that may be at risk. Prepare written monthly activity reports for leadership. Participate in staff meetings, workgroups, and assigned projects.

Minimum Qualifications Candidates must have:

At least one year of professional experience developing, implementing, or evaluating programs involving human services, education, sociology, public health, healthcare services, or healthcare insurance. A Bachelor's degree in a Human Services or Education field , OR an equivalent combination of relevant education, training, and professional experience. Knowledge of resources available to low-income residents . Familiarity with human-services governmental operations at the federal, state, county, and local levels . Intermediate proficiency with Microsoft Office Suite and technical computer software. General research and online research skills. Strong written and verbal communication skills. Ability to work effectively as part of a team or workgroup. Strong troubleshooting and problem-solving abilities. Ability to analyze and interpret complex or technical information. Ability to establish and follow procedures and controls to ensure program objectives are met.
