Experience Proven experience in clinical case management within hospital medicine, emergency medicine, or post-acute care environments. Demonstrated expertise in critical care settings such as Level I or Level II trauma centers, ICU, or infectious disease units. Familiarity with hospital experience including medical records management, inpatient assessment procedures, and acute care protocols. Prior involvement in discharge planning, home health coordination, hospice care, or managed care programs is highly desirable. Strong knowledge of healthcare policies related to HIPAA compliance, medical terminology, and health regulation standards. Experience working with electronic health records (EHR) systems and managing patient data securely. Ability to communicate effectively with diverse healthcare teams and patients from varied backgrounds in a fast-paced environment.
