Job Description
Care Team Coordinator The Care Team Coordinator serves as an administrative and regulatory coordination resource for the interdisciplinary team (IDT), ensuring compliance with PACE regulations and organizational standards. This role coordinates Service Determination Requests (SDRs), Plan of Care (POC) meetings, documentation, and required assessments while supporting efficient communication among the interdisciplinary team. The IDT Coordinator promotes regulatory compliance, quality improvement, and timely care planning to ensure exceptional participant outcomes. What is PACE? PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states. The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.
