(MMH) is seeking a Chronic Disease Self-Management Program (CDSMP) Coordinator to coordinate the implementation and day-to-day operations of a three-year statewide initiative serving older adults and adults with disabilities throughout Connecticut. This position The Coordinator will oversee program scheduling, participant recruitment and enrollment, facilitator coordination, community partnerships, program fidelity, data collection, quality assurance, and performance monitoring. The position will also support implementation of the Program to Encourage Active, Rewarding Lives (PEARLS) as an integrated component of the project. Over the three-year grant period, MMH plans to expand CDSMP statewide, serve approximately 2,800 CDSMP participants , integrate PEARLS services for eligible participants, train additional program leaders, and strengthen statewide community and healthcare referral networks.
IMPORTANT
This position may be full-time or part-time . Full-time Coordinators are expected to dedicate approximately 10 hours per week to direct delivery of community-based health promotion programs . Part-time responsibilities and compensation will be adjusted based on the agreed-upon schedule. Qualifications Required Bachelor's degree Minimum of 2 years of experience coordinating community-based health promotion, chronic disease prevention or management, or similar health programs . Minimum of 2 years of experience implementing or coordinating Diabetes Self-Management Education and Support (DSMES), or Chronic Disease Self-Management Program (CDSMP). Proficiency with Microsoft Office and electronic data-management systems. Strong organizational, project coordination, and time-management skills. Strong written and verbal communication skills. Ability to collect, maintain, and review program data and performance measures. Ability to travel throughout Connecticut. Preferred Registered Dietitian (RD/RDN) or Registered Nurse (RN). Experience working with older adults and adults with disabilities. Experience with federally funded health programs, grant reporting, quality assurance, or program evaluation. Bilingual English/Spanish skills.
Essential Duties and Responsibilities:
Recruit, onboard, train, schedule, and provide ongoing support to CDSMP Peer Leaders and other program facilitators. Establish and maintain relationships with community host sites and coordinate workshop logistics, including space, schedules, supplies, materials, and facilitator assignments. Coordinate outreach activities with Area Agencies on Aging, senior centers, assisted living facilities, healthcare providers, community-based organizations, and other statewide partners. Manage the daily operations and implementation of the CDSMP program in accordance with the approved grant work plan. Coordinate the scheduling and delivery of CDSMP workshops in in-person, virtual, and hybrid formats throughout Connecticut. Coordinate with Master Trainers to facilitate CDSMP leader training and maintain sufficient facilitator capacity. Recruit and enroll eligible participants and monitor progress toward annual enrollment and completion goals. Coordinate PHQ-9 screening and referral processes for CDSMP participants who may be eligible for PEARLS services. Work closely with the Case Manager to ensure participants with identified social needs are appropriately referred to community resources. Monitor CDSMP program fidelity and quality, ensuring Peer Leaders and facilitators follow established protocols and evidence-based program requirements. Conduct site visits and quality-control monitoring and provide technical assistance and support to facilitators. The approved project specifically assigns the Program Coordinator responsibility for fidelity monitoring, with reporting to the Program Director. Track participant enrollment, attendance, retention, completion, referrals, and program outcomes. Administer and coordinate participant surveys and other program evaluation activities. Maintain accurate and complete program records and ensure timely entry of required program data. Assist with collection, analysis, and reporting of grant performance data, including attendance, pre- and post-surveys, PHQ-9 screenings, SDOH assessments, and referral tracking. Monitor progress toward monthly, quarterly, annual, and three-year program goals and identify areas requiring corrective action. Assist with preparation of required grant reports and documentation. Support the development and review of accessible program materials, including materials meeting Section 508 accessibility requirements. Participate in project meetings, partner meetings, required trainings, technical assistance sessions, and other grant-related activities. Travel throughout Connecticut to monitor program sites, support facilitators, engage partners, and ensure consistent program implementation. Provide regular program updates to the Program Director and promptly communicate significant implementation or performance issues. Perform other duties related to successful implementation of the grant-funded program.
