Position Summary Monitor My Health, Inc. (MMH) is seeking a Mobile Clinic Program Coordinator to oversee the implementation and delivery of two community-based preventive health programs throughout
Connecticut:
an A1C-focused Mobile Clinic , providing A1C testing and blood pressure screenings, and a Balance and Fall Prevention Clinic , providing fall-risk and balance assessments for older adults through community-based screening events and individual in-home hazard assessments . The Coordinator will oversee the day-to-day operations of both programs, including staff supervision, clinic scheduling, community outreach, screening services, participant follow-up, data management, and program reporting . The Coordinator will also organize community screening events, oversee in-home fall-risk and home-safety assessments for older adults , and supervise and support Mobile Clinic staff. This position may be full-time or part-time . Full-time Coordinators will combine program coordination and supervision with direct delivery of Mobile Clinic services. Qualifications Required Healthcare background and applicable professional credentials, such as Registered Nurse (RN), Registered Dietitian/Registered Dietitian Nutritionist (RD/RDN), Occupational Therapist (OT), or another relevant healthcare professional. Minimum of 2 years of program coordination or related healthcare/community health experience . Experience working in community-based health promotion, preventive health, chronic disease prevention, older adult services, or a related setting. Ability to perform or be trained to perform point-of-care A1C testing, blood pressure screening, and fall-risk/balance assessments within applicable scope and program requirements. Experience coordinating staff, schedules, community events, and multiple program activities. Strong organizational, communication, and documentation skills. Ability to collect, maintain, analyze, and report program data. Ability to travel throughout Connecticut and conduct services in community and residential settings. Valid driver's license and reliable transportation. Preferred Experience working with older adults and diverse or underserved populations . Bilingual English/Spanish skills. Essential Duties and Responsibilities Coordinate the day-to-day operations of two Mobile Clinic programs: an A1C-focused clinic and a balance/fall-risk assessment clinic . Recruit, hire, train, schedule, and supervise Mobile Clinic staff , providing ongoing training and support. Conduct outreach and clinic solicitation and develop partnerships with senior centers, housing sites, healthcare providers, community-based organizations, and other partners to secure screening locations and schedule events. Manage clinic schedules and logistics, ensuring adequate staffing, supplies, equipment, and materials for all scheduled events. Manage program supply logistics, including A1C tests, screening equipment, forms, sharps/needle disposal, and other required supplies . Perform A1C testing, blood pressure screenings, and balance/fall-risk assessments and serve as a screening technician as needed. Provide on-site staff supervision, ongoing training, and quality assurance to ensure screening, safety, and program protocols are consistently followed. Design, coordinate, and implement the in-home fall-risk and home-safety assessment component for older adults, including scheduling, assessments, follow-up, and referrals. Oversee participant records, data entry, data quality, statistical analysis, and program outcome tracking for both clinic programs. Manage the Care Navigation/SDOH component , including weekly review of participant progress and referrals, identification and maintenance of community resources, and participant outreach at clinic events. Coordinate Diabetes Prevention Program (DPP) recruitment and referrals for participants with A1C results in the prediabetes range and work with the DPP Program Coordinator to align screening events with available DPP classes. Prepare required monthly, quarterly, annual, and grant-related program reports for organizational leadership, funders, and other stakeholders and monitor progress toward program goals. Supervise the Program Assistant , including work related to social media and program website management, data entry, outreach materials and flyers, and clinic solicitation. Travel throughout Connecticut to coordinate, conduct, and supervise community screening events and in-home assessments .
