Immediately Job Description Student Program Coordinator will be the primary contact for the parent/guardian, school-based team members, funding agencies and outside providers. The Student Program Coordinator will work as part of the student's team to gather, share, and hold all information about a student's care and needs.- Responsible for overall communication with parent/guardian, school district and outside agencies.
- Meet with potential new students and families to transition them to our school in coordination with the Director of Admissions
- Facilitate internal meetings including: internal team collaboration, progress, transitional, service planning, etc.
- Day-to-day coordination across environments on an as needed basis to allow students to successfully access their educational program
- Available, flexible, responsive in order to support the student, staff, and family's needs
- Ensures that information regarding student's safety and welfare are communicated to parents and all team members (i.e. behavioral incidents, medical incidents, programmatic changes)
- Communicate regularly with all staff in order to ensure that student needs are being met across all settings
- Problem solves any arising issues with the team. Reports concerns and plans to parent.
- Will serve as a resource to families through transition process (i.e. 688 process, guardianship, Rogers, etc.) in order to access appropriate services within the necessary time frame.
- Collaborate with outside agencies/families in all aspects of the transition process to other programs or reintegration to program (i.e. return from inpatient hospitalization, move to less restrictive environment, internal program transfer, transfer to adult services etc.)
