POSITION SUMMARY
We are currently recruiting for a Human Services Specialist IV - Tobacco & Chronic Disease Program Specialist to support the Montgomery County Department of Health and Human Services (DHHS), Public Health Services, Cancer, Tobacco & Cannabis Prevention Programs. The Human Services Specialist IV will support tobacco prevention and cessation initiatives, chronic disease and diabetes self-management programs, community outreach, grant deliverables, program evaluation, and health education activities. The successful candidate will work closely with community partners, healthcare organizations, and internal stakeholders to implement and evaluate evidence-based public health programs. This is a full-time, grant-funded position working 40 hours per week and is anticipated to continue through June 30, 2027. The position is based in Rockville, MD with telework eligibility. The pay range for this position is $25.00-$27.00 per hour.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Coordinate and support tobacco use prevention, cessation, and education activities in collaboration with community organizations, healthcare providers, hospitals, and other partners. Coordinate, facilitate, implement, and monitor Diabetes Self-Management Programs (DSMP), Chronic Disease Self-Management Programs (CDSMP), and related evidence-based chronic disease interventions. Support tobacco cessation programs and resources, including Freedom From Smoking programs and referrals to available quit-support services. Promote public awareness and education regarding tobacco prevention, vaping prevention, chronic disease management, cancer prevention, and other public health initiatives. Develop and support social marketing campaigns, health education materials, flyers, presentations, and other community-facing educational resources. Track program activities, participation, outcomes, performance measures, and other data required to evaluate program effectiveness. Monitor grant deliverables and assist with preparing required program reports, documentation, statistics, and performance updates. Evaluate the effectiveness of tobacco prevention, chronic disease, health promotion, and community education programs and assist with the development and administration of evaluation tools. Build, develop, and maintain productive relationships with internal and external community partners. Coordinate and participate in community outreach initiatives, educational events, partner trainings, and other public health activities. Support cancer prevention and screening outreach efforts, including education regarding available colorectal cancer screening resources. Support community education related to tobacco and nicotine products, vaping, cannabis prevention, and other emerging public health concerns. Assist with retailer education and tobacco compliance initiatives related to applicable local and state requirements. Assist in the preparation of grant applications, reports, and related materials, including researching and compiling background information, program data, and statistics. Maintain accurate program records and ensure activities and outcomes are properly documented. Collaborate with team members and community stakeholders to ensure grant-funded programs meet established objectives, timelines, and deliverables. Perform related duties as assigned.
REQUIRED SKILLS/ABILITIES
Strong knowledge of or experience with public health, human services, community health, social services, community outreach, advocacy, or related programs. Ability to coordinate multiple programs, activities, deadlines, and priorities simultaneously. Experience collecting, tracking, analyzing, and reporting program data and outcomes. Strong organizational skills and exceptional attention to detail. Excellent verbal, written, interpersonal, and listening communication skills. Ability to establish and maintain effective working relationships with community organizations, healthcare providers, government agencies, and other stakeholders. Ability to communicate health information clearly to diverse community populations. Ability to assist with the development of educational, outreach, and social marketing materials. Ability to work independently and collaboratively as part of a public health team. Self-motivated with the ability to take initiative and follow through on assigned responsibilities. Ability to understand and adhere to the duties, methods, procedures, policies, and grant requirements associated with the position. Proficiency with Microsoft Office Suite and comfort learning and utilizing technology and data-tracking systems. Bilingual English/Spanish skills are preferred but not required.
EDUCATION AND EXPERIENCE
Candidates must meet one of the following combinations of education and experience: Bachelor's degree and at least three (3) years of relevant professional experience in human services, public health, eligibility programs, social work, community outreach, advocacy, or a related field; OR At least five (5) years of relevant professional experience in human services, public health, eligibility programs, social work, community outreach, advocacy, or a related field without a bachelor's degree.
PREFERRED EXPERIENCE
The ideal candidate will have experience in one or more of the following areas: Public health program coordination, health education, or community health work. Coordinating or facilitating Diabetes Self-Management Programs (DSMP) or Chronic Disease Self-Management Programs (CDSMP). Evidence-based programs such as Stanford's Better Choices, Better Health or similar chronic disease self-management interventions. Tobacco prevention and cessation initiatives, including Freedom From Smoking or similar programs. Grant-funded program implementation, monitoring, data collection, reporting, and evaluation. Community partnership development and stakeholder engagement. Planning or participating in community outreach events and health education initiatives. Creating flyers, presentations, social marketing content, and public health education materials. Working with diverse communities and developing or delivering bilingual health education materials.
PHYSICAL REQUIREMENTS
Prolonged periods of sitting at a desk and working on a computer. Ability to perform standard office and community-program-related duties associated with the position.
