Job Description
Job Summary We are seeking a dynamic and dedicated Program Coordinator to lead and manage our Residential care homes within long-term care and HCBS. In this vital role, you will coordinate comprehensive care programs, oversee team operations, ensure compliance with health regulations, and foster a compassionate environment for residents. Your leadership will drive quality care delivery, enhance resident satisfaction, and support the overall success of our care services. This position offers an exciting opportunity to make a meaningful difference in the lives of adult with with developmental disabilities through effective program management and team supervision. Duties Develop, implement, and monitor individualized care plans tailored to residents' needs, including those requiring Alzheimer's, dementia, or memory care. Supervise care facility teams, including nursing staff, social workers, and support personnel, ensuring high standards of resident care and staff performance. Manage daily operations of the facility, including staffing schedules, budgeting, and resource allocation to maintain efficient service delivery. Ensure compliance with long-term care regulations, health policies, and safety standards across all programs. Oversee medication administration processes and coordinate with nursing leadership to uphold medication safety protocols. Maintain accurate documentation related to resident care, staff activities, incident reports, and regulatory audits using Microsoft Office tools. Collaborate with families, healthcare providers, and community resources to support residents' well-being and transition planning. Experience Extensive experience working in nursing homes or assisted living facilities with a focus on senior or long-term care. Proven supervisory or management experience within healthcare or residential care settings. Strong knowledge of dementia care, Alzheimer's disease, memory care strategies, and resident's (personalized resident information). Familiarity with long-term care regulations, health regulation policies, and compliance standards. Demonstrated ability to develop and manage care plans tailored to individual needs. Proficiency in Microsoft Office Suite for documentation and reporting purposes. Experience working with individuals with developmental disabilities or in disability services is highly desirable. Background in healthcare financial management, budgeting, and operational oversight within a residential or community-based setting. Leadership skills demonstrated through supervising teams in senior living or group home environments. Knowledge of medication administration procedures and home & community care practices.
