Healthcare Staffing Professionals has an immediate need for a Health Program Coordinator. HPC duties must comply with established Department of Public Health (DPH) policies procedures, state and local laws, ordinances, and regulations for the control of communicable disease. HPC must maintain client confidentiality requirement per the Health Insurance Portability Accountability Act (HIPAA). In addition, staff must adhere to LAC DPH, and Protected Health Information, procedure, policies, and ordinance pertaining to HIV Security and Confidentiality laws and trainings. The Division of HIV and STD Programs (DHSP) Health Program Coordinator (HPC) plans, coordinates, and implements public health programming by providing direct services in the field, developing and monitoring management and logistical processes, assists Public Health Investigators (PHIs) for the control of communicable diseases with special focus on HIV and STIs. Assignments may include but not limited to linking clients to HIV care and syphilis treatment, conducting specialized CDR interviews with clients, transporting clients to high priority medical appointments, conducting chart reviews, providing Field Delivered Therapy (FDT), collaborating with other Direct Community Services (DCS) programs to implement field based/mobile testing services. The HPC supports data management efforts including data entry, data quality assurance, analyzing HIV and syphilis labs, and collaborating with surveillance units. The HPC will lead planning and logistics for outreach events for special initiatives and DCS programs. The positions will start immediately upon acceptance and employment clearance and are initially funded through 8/31/27, with the good chance of extension.

$30.24/hr working 40 hours a week

Initial Job Duties:

Activities include but are not limited to conducting linkage to care, treatment, biomedical prevention, or other essential services for priority clients/contacts; conduct Cluster Detection and Response (CDR) enhanced re-interviews; support case investigation and documentation; analyze HIV and STI case information to develop and implement timely disease intervention plans; complete treatment verification and chart reviews; counsel and educate clients on risk-reduction and prevention strategies; provide FDT and other supplies such as self-test kits, condoms, harm reduction items. Develop tracking plans, service reports, and scopes of work to ensure goals are met. Coordinate outreach activities to support DCS program efforts. Lead logistical activities to prepare and report event outcomes. Support testing events as the mobile unit driver, on-site coordinator, working with venue partners. Qualifications:

Four years of experience in planning and implementing organized community programs and activities in public or community health, delinquency prevention, community relations, or community welfare. Graduation from an accredited college or university with a degree in public health will be accepted for one year of the required experience. A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions. Thorough knowledge of HIV and STI testing, biomedical prevention, and treatment protocols. Knowledge of HIV/STI, drug treatment, mental health, housing service programs. Ability to complete HIV/STI counseling and testing training and certification. Knowledge of and experience with HIV/STI labs, data, data systems, and quality assurance standards. Strong outreach skills. Strong organizational and interpersonal skills. Excellent leadership and communication skills (verbal and written). Knowledge with Microsoft Word, Access, Excel, Power-point, and ability to learn internal data systems. Able to participate in field-based services.
