Homecare Scheduler Schedule :
Full-time, 40 hours per week | Monday- Friday | 8am

The Homecare Scheduler will be responsible for scheduling of homecare services for PACE participants. Department Participant Care Employment Type Full Time Location Jackson, MI Workplace type Onsite Compensation $19.00- $20.

staff. Maintains homecare scheduling software PACECare 365 which includes but not limited to daily/weekly. scheduling, revamping schedule, and communication with family and participants. Communicates with participant care manager regardingCENA/PCA
pick-up and trades. Maintains service notes within the EMAR related to services provided to participants. Works with contracted agencies to secure in-home staffing as needed. Works with aide staffing with communication of care needs, scheduling questions or concerns. May assist in participant homes when staff is needed. Demonstrates active participation in the QI process such as chart reviews. Must be able to read, write, speak and understand the English language and follow written and oral instructions. Must be CPR certified or willing to obtain. At least one (1) year of experience working with the elderly.
