Role:
As a Hospice Home Health Scheduler, you will play a critical role in coordinating and scheduling patient care services, ensuring seamless transitions between hospital and home settings. Your attention to detail and organizational skills will enable you to prioritize patient needs, manage complex schedules, and communicate effectively with healthcare teams. Responsibilities:
Coordinate patient care services, including scheduling home visits, hospital transfers, and other medical appointments Manage patient schedules, ensuring timely and efficient delivery of care services Collaborate with healthcare teams to identify patient needs and develop personalized care plans Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions Utilize electronic medical records (EMRs) and scheduling software to track patient data and schedules Identify and address potential scheduling conflicts or issues, escalating concerns as necessary
Requirements:
