Job Description

UPMC Home Health is hiring a full-time Scheduling Coordinator for the West Mifflin Office The Home Health Scheduling Coordinator is an on-site position that coordinates clerical and scheduling functions for the multidisciplinary team under the direction of the Clinical Manager. This is a hybrid role- must be able to work on-site in the West Mifflin office
Hours:
Monday-Friday 8:00 am-4:30 pm with rotating weekends and holidays
Responsibilities:
Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures. Focus on customer service and continually strive to perform the duties of their job in a manner that results in optimal patient satisfaction. Identify, develop, and participate in process improvement opportunities within the home health agency that will enhance the quality of the services provided. Perform duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC toward patients, visitors, staff, peers, physicians, and other departments within the organization. Completes payroll duties as assigned Calls all new patients for an Admission visit or Resumption of Care visit Performs visit verification activities daily. Verifies visits provided to visits ordered, scheduled, documented, and notifies the Clinical Manager of discrepancies Responsible for maintaining the team master schedule and visit verification Performs scripted Welcome Calls and Outreach Calls to all new patients Coordinates with Clinical Manager and Intake to schedule all patient admissions and initial visits for internal multidisciplinary referrals Coordinates scheduling of homecare patient visits based on patient needs, productivity expectations, and staffing, with the assistance of the Clinical Manager Process, reconcile, and distribute patient information as needed Accept patients, family, and staff telephone calls and facilitate communication between team members. Update patient information, print patient information and reports as required and/or directed. High school diploma or GED required. Scheduling and coordinating experience preferred 1-year related work experience preferred. Competency with PC preferred. Knowledge of medical terminology preferred.
