We are currently seeking a Home Health Aide (HHA) or Certified Nurse Assistant (CNA) to join our Home Care and Private Duty Services team. In this role, you will be a vital part of a patient-centered care team, providing support to patients in their homes while collaborating with families and interdisciplinary healthcare providers to ensure high-quality care and positive outcomes. Key Responsibilities:

Provide direct patient care in the home setting under the direction of the care team Assist patients with activities of daily living and personal care needs Communicate effectively with patients, families, and healthcare team members Monitor and report changes in patient condition or progress Maintain accurate documentation of care provided Ensure patient safety and follow all care plans and protocols

QUALIFICATIONS

Education:

High school diploma or equivalent required

Experience:

Minimum of 1 year of healthcare experience preferred

Knowledge & Skills:

Ability to work independently and take initiative Strong time management and organizational skills Demonstrates accountability and responsibility for safe patient care Effective communication and interpersonal skills

Licensure, Registration, or Certification:
