We are currently seeking a Social Worker Case Manager to join our team. In this role, you'll provide high-quality, patient-centered case management services that support positive outcomes for patients, families, physicians, and staff. As a Social Worker Case Manager, you'll play a key role in coordinating care and discharge planning by applying strong clinical judgment, communication, and problem-solving skills. You'll work collaboratively with internal teams and external agencies to ensure patients receive appropriate care and resources while maintaining a professional standard in all case management activities.Key responsibilities include:
Coordinating patient care and discharge planning to support optimal outcomes Collaborating with physicians, patients, families, and interdisciplinary teams Communicating effectively with insurance providers and community resources Applying critical thinking to assess, plan, implement, and evaluate care management services Demonstrating strong organizational, multitasking, and interpersonal skills Maintaining professionalism while working independently and as part of a teamQUALIFICATIONS
Education:
Social Worker with Bachelor's degree requiredExperience:
Three to five years of hospital experience preferred Experience in case management or a related field strongly preferred Experience in rehabilitation settings is a plusKnowledge & Skills:
Knowledge of care management, resource utilization, and utilization management required Strong critical thinking, communication, and organizational skills Ability to manage multiple priorities in a fast-paced environment Commitment to providing high-quality, patient-centered service Licensure, registration or certification: Current licensure in the State of Kansas required Basic Life Support (BLS) certification required Specialty certification strongly encouraged within three years Join our Team!
