Job Description

Position Summary:
The Clinical Social Worker provides support and case management services for the Exceptional Family Member Program - Medical (EFMP-M) at Cannon AFB . The Social Worker oversees and manages EFMPM operations in accordance with Department of Defense (DoD) and Air Force (AF) policies while coordinating care, outreach, referrals, and support services for military families with special medical and educational needs. Services are delivered within a Military Treatment Facility (MTF) environment in support of operational readiness and family support initiatives.
Compensation & Benefits:
Pay Rate:
$45.00/hr. to $48.00/hr. Health & Welfare (H&W): $5.09/hr. Paid Time Off (PTO): 80 hours accrued annually
Sick Leave:
56 hours accrued annually
Holidays:
11 paid federal holidays annuall
Required Qualifications:
Education:
Degree/Education requirements in accordance with Technical Exhibit (TE) 5 of the PWS.
Experience:
Minimum of two (2) years of full-time experience within the last four (4) years in care coordination, discharge planning, or case management in a medical setting.
Licensure:
Must meet licensure requirements in accordance with the base contract PWS and TE 5. Position is not designated as Licensed Clinical Social Worker (LCSW)-required by the local Medical Group.
Certifications:
Current Basic Life Support (BLS) certification required. Additional certifications as required by TE 5. Legal Authorization Must be a U.S. Citizen.
Work Schedule & Coverage:
Monday through Friday schedule. Eight-hour shifts between 0700-1700. Maximum of 40 hours per week excluding federal holidays. One-hour non-billable lunch period. Services are considered mission essential. No on-call hours require
Key Responsibilities:
Clinical & Program Duties:
Oversee and manage the installation EFMP-M program in accordance with DoD and Air Force guidance. Identify sponsors and family members with special medical and educational needs using clinical assessment skills and multidisciplinary collaboration. Ensure military family members are appropriately enrolled and "Q coded" in EFMP systems. Maintain oversight of data entry and management within Air Force medical special needs systems. Coordinate enrollment and travel screening for active-duty family members and overseas assignments. Maintain Special Needs files at the Medical Treatment Facility (MTF) and conduct annual outreach to enrolled families. Participate in EFMP quarterly case reviews to assess family needs and determine required resources. Coordinate referrals to Airman & Family Readiness Centers and civilian/community agencies as appropriate. Collaborate with Integrated Delivery Systems (IDS), personnel offices, and support agencies to ensure continuity of care and family support. Maintain accurate medical record documentation and required reporting in GENESIS and applicable systems. Participate in orientation and training activities for EFMP and Special Needs Program personnel. Serve on committees, work groups, and task forces as assigned. Quality & Compliance Maintain productivity and quality standards consistent with professional social work practices and DoD guidance. Complete all required electronic medical record training and MTF-specific orientation programs. Follow all applicable MTF, DHA, AF, and DoD policies and procedures.
