Client Care Manager | Home Care Operations & Scheduling Full-Time | Salaried, Exempt | On-Call Rotation Required About the Role We are seeking an experienced, highly organized Client Care Manager to oversee client care, caregiver performance, scheduling continuity, and daily home care operations. This is a hands-on leadership position for someone who is comfortable making decisions, solving problems, and working in a fast-paced environment. The Client Care Manager works closely with clients, families, caregivers, Scheduling, HR, Billing, and leadership to ensure clients receive dependable, high-quality care. This position requires participation in an after-hours, weekend, and holiday on-call rotation. Key ResponsibilitiesClient Care & Operations Serve as a primary point of contact for clients and families. Respond to caregiver call-outs, care concerns, changes in condition, and service disruptions. Communicate with families and resolve concerns promptly. Conduct client visits, caregiver supervisory visits, and caregiver check-in calls as required. Update care plans and communicate changes to caregivers and appropriate team members. Address and document client and caregiver concerns or complaints. Maintain accurate client, caregiver, and operational documentation. Scheduling & Service Continuity Oversee upcoming schedules and help ensure shifts remain appropriately staffed. Review open shifts, caregiver availability, permanent schedules, and staffing needs. Make appropriate caregiver assignments or schedule adjustments when needed. Monitor overtime, caregiver utilization, time-off requests, and travel time between clients. Collaborate with Scheduling and Recruiting to identify current and upcoming staffing needs. Caregiver Performance & Compliance Monitor caregiver attendance, clock-ins, clock-outs, and documentation. Address performance or compliance concerns. Provide coaching, education, and corrective action when appropriate. Help coordinate mentorship and support for new caregivers. Take appropriate action when client safety, caregiver performance, or company policy is at risk. Billing & Documentation Review schedules for missed punches, overlaps, mileage, and other discrepancies. Assist with daily billing cleanup and documentation. Identify and resolve scheduling, billing, and pay-rate discrepancies. Maintain accurate reports, logs, schedules, and activity notes. On-Call Responsibilities Participate in the agency's on-call rotation. Respond to urgent caregiver call-outs and coverage needs. Handle after-hours client, family, and caregiver concerns. Resolve urgent service disruptions and safety concerns. Document on-call activity and communicate necessary follow-up to the team. Qualifications & Requirements High school diploma or GED required. Previous experience in home care, healthcare, staffing, scheduling, client services, or a related service-based environment preferred. Previous leadership or supervisory experience preferred. Valid driver's license. Reliable transportation and current auto insurance. Ability to travel locally for client and caregiver visits as needed. Strong communication, customer service, and problem-solving skills. Excellent organization and time-management skills. Ability to manage multiple priorities and make sound decisions independently. Comfortable using scheduling software, email, reports, and other technology. Ability to participate in an after-hours, weekend, and holiday on-call rotation. Ability to successfully complete required background and employment screenings. Home care operates around the clock, so the Client Care Manager must be comfortable addressing urgent situations and participating in the agency's on-call rotation, including assigned evenings, weekends, and holidays.
