Job Description

Acara Health, one of the largest providers of quality in-home health care in the state of Texas, is seeking an innovator and leader as a Care Coordinator. Acara Health's Personal Care provides industry-leading in-home care in the markets of Central and South Texas. We are growing and adding positions to our team! The job duties for the position of Care Coordinator in Laredo are listed below. The Care Coordinator will work closely with the client and home care aid to: Coordinate client service requests and ensure timely follow-up to meet client needs. Answer and manage telephone inquiries from current and prospective clients in a professional and courteous manner. Completing required service-related follow-up and reports within established deadlines. Ability to manage a high-volume caseload, prioritize multiple deadlines, and follow through on outstanding items. Address client concerns and complaints, escalating issues to the appropriate personnel when necessary. Recruit, screen, interview, and coordinate new hire onboarding, orientation, and ongoing support for field staff. Schedule and manage field staff assignments to ensure adequate client coverage and service continuity. Maintain accurate and up-to-date records of active clients, caregivers, schedules, and personnel files. Perform administrative duties, including preparing reports, reviewing documentation, processing timesheets, and maintaining office records. Communicate regularly with supervisors, clients, and field staff to ensure quality service and operational efficiency. Support staff development, training, quality improvement initiatives, and other departmental responsibilities as assigned. Monitoring EVV or timekeeping compliance and following up on missing or incorrect visits.
Qualifications/Experience:
Must be at least 18 years of age. Highschool Diploma or GED Required Excellent computer skills, including proficiency in Microsoft Office applications, email, data entry, and other office technology systems. Possesses basic knowledge of personal care practices and household maintenance. Demonstrates punctuality, dependability, and a strong commitment to reliability.
Bilingual:
Fluent in both English and Spanish, with the ability to communicate effectively in each language. Provides evidence of competency through relevant education, training, and/or work experience, or demonstrates the ability to successfully perform assigned duties. Self-motivated, enthusiastic, and detail-oriented professional with strong organizational skills and the ability to manage multiple priorities while meeting deadlines.
Skills:
Ability to communicate effectively and keep information confidential. Must be able to follow written and verbal instructions and is competent to perform tasks assigned by supervisor. Demonstrates interest in the welfare of ill, elderly, and/or disabled. Ability to remain organized while managing multiple details. Maintain calm presence during periods of high call volume or unusual events. Professional and customer service driven phone presence.
