The Client Advocate will serve as an important point of contact for prospective and current clients, their families, facilities, caregivers, and our internal team. This position is responsible for responding to leads, conducting client assessments, completing environmental and safety checks, coordinating follow-up, maintaining strong client relationships, and ensuring that families and referral partners receive timely updates. The ideal candidate is personable, professional, dependable, detail-oriented, and comfortable working independently while managing multiple clients and priorities. Key Responsibilities Lead Management & Intake Respond promptly and professionally to new client leads and inquiries. Build rapport with prospective clients, families, referral sources, and facilities. Gather information regarding the client's needs, preferences, schedule, and desired services. Follow up consistently with leads throughout the intake process. Help convert appropriate inquiries into new clients while providing a positive experience from the first point of contact. Client Assessments Conduct initial client assessments to understand the client's needs, routines, preferences, and service expectations. Identify appropriate non-skilled private duty services based on the client's needs and agency capabilities. Document assessment information accurately and communicate relevant details to the appropriate team members. Coordinate with families and referral sources to ensure expectations are clearly understood. Environmental & Safety Checks Conduct home and/or facility visits as needed. Complete environmental checks to identify potential safety concerns, accessibility issues, or other factors that may affect service delivery. Communicate concerns to the appropriate team members and help coordinate solutions when needed. Ensure the client's environment is appropriate for safe and effective non-skilled care. Client & Family Relations Serve as a consistent point of contact for clients and families. Maintain regular communication with families regarding services, concerns, changes, and client needs. Respond to questions and concerns in a timely, professional, and compassionate manner. Help resolve service-related concerns and escalate issues when appropriate. Build long-term relationships with clients, families, and referral partners. Facility & Referral Partner Communication Maintain positive relationships with assisted living communities, facilities, discharge planners, referral partners, and other community contacts. Provide appropriate updates regarding clients and services. Coordinate communication between facilities, families, caregivers, and agency leadership. Support the agency's efforts to develop and maintain referral relationships. Internal Communication & Coordination Provide regular updates to the Client Relations Coordinator regarding leads, assessments, client concerns, service changes, and follow-up needs. Communicate important client information to the appropriate members of the care and administrative team. Monitor outstanding follow-ups and ensure tasks are completed in a timely manner. Help identify potential client satisfaction issues before they become larger concerns. Qualifications Previous experience in home care, senior care, healthcare, client relations, case coordination, social services, or a related field preferred. Excellent communication and interpersonal skills. Strong organizational and follow-up skills. Ability to build trust and rapport with clients and families. Comfortable conducting home or facility visits. Professional, compassionate, and solutions-oriented approach. Ability to manage multiple clients, leads, and priorities. Strong attention to detail and documentation skills. Reliable transportation and ability to travel locally as needed. Ability to work independently while maintaining strong communication with the agency team.
