With offices in Northborough and Natick, Massachusetts , we serve clients throughout Greater Worcester and the MetroWest area. We are looking for a compassionate, organized, and relationship-focused Client Care Coordinator to join our team. This is a great opportunity for someone with experience or education in home care, senior care, social work, psychology, human services, case management, or a related field who enjoys working directly with clients and families. The Client Care Coordinator helps new and existing clients navigate their care needs from the initial consultation through ongoing service. This role includes client assessments, developing individualized care plans, introducing Care Professionals to clients, completing quality assurance visits, and maintaining strong relationships with clients and families.
Schedule:
Full-time, Monday-Friday, 9:00 AM-5:00 PM. Flexibility outside of normal business hours may occasionally be required.
Location:
This position is based out of our Northborough and Natick offices and r equires frequent local travel to client homes throughout Greater Worcester and MetroWest. All business mileage is reimbursed. What You'll Do Conduct client consultations and assessments with prospective clients and their families. Educate families about Home Instead services and recommend solutions based on each client's individual needs. Develop personalized plans of care and ensure all required documentation is completed accurately and promptly. Coordinate introductions between clients and Care Professionals. Help prepare Care Professionals for assignments by communicating client needs and offering additional training when appropriate. Conduct ongoing quality assurance visits with clients and families. Monitor client satisfaction and changes in care needs. Update client records, plans of care, and documentation as services or conditions change. Communicate regularly with clients, families, Care Professionals, referral partners, and internal team members. Identify opportunities to adjust services when a client's needs increase or change. Help resolve client or family concerns and ensure appropriate follow-up. Support client retention by building strong, trusted relationships with the families we serve. What We're Looking For Associate degree in social work, psychology, human services, healthcare, or a related field preferred; equivalent experience will also be considered. At least 1 year of experience in home care, senior care, healthcare, social services, case management, or a related field preferred. Strong communication and relationship-building skills. Compassionate and professional approach when working with older adults and families. Ability to manage multiple priorities in a fast-paced environment. Strong writing, documentation, and problem-solving skills. Comfortable using computers, Microsoft Office 365, and electronic record systems. Valid driver's license and ability to travel locally to client homes. Why Join Home Instead?
