In this role, you'll coordinate intake, scheduling, referrals, and follow-up for complex and chronic care patients, ensuring timely access to services and benefits. You will review clinical information, document interactions, and collaborate closely with nurses, pharmacists, and providers to close care gaps and improve outcomes. The role requires strong communication, EMR data entry, customer service skills, and a passion for helping diverse populations navigate healthcare resources. Responsibilities Coordinate member intake, benefits verification, referrals, and follow-up appointments. Support nurses, pharmacists, and providers with care management activities. Review and document clinical and utilization information in EMR and related systems. Perform outreach calls to members to close care gaps and improve adherence. Educate members on available programs, benefits, and community resources. Maintain HIPAA compliance and protect member confidentiality. Monitor task queues and prioritize urgent or high-risk cases. Track and report on member status, barriers, and next steps. Collaborate with internal teams to streamline workflows and improve outcomes. Contribute to a culture of inclusion, integrity, and continuous improvement.
