In this energetic and rewarding position, you will provide personalized assistance, ensuring safety, promoting well-being, and fostering community engagement. Responsibilities Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals or supervisors promptly. Administer medications following care plans and HIPAA guidelines to ensure safety and compliance. Prepare nutritious meals and snacks tailored to individual dietary needs while maintaining cleanliness and food safety standards. Support residents with community outings, social activities, and recreational programs to promote engagement and independence. Maintain a safe environment by adhering to infection control protocols, performing cleaning duties, and ensuring the proper use of safety equipment. Document care activities accurately in resident records, including progress notes, medication logs, and incident reports. Collaborate with team members to develop and implement personalized care plans that reflect each resident's unique needs and goals. Use patient observation skills to detect early signs of health issues Support residents' emotional well-being through compassionate communication, active listening, and engaging in meaningful interactions. Follow all safety protocols related to CPR, first aid, hospice care procedures, and other emergency response measures to ensure resident safety at all times. Skills Proven experience working in assisted living facilities, nursing homes, group homes, or home & community care environments. Strong knowledge of patient care practices including medication administration, vital signs monitoring, dementia care, memory care, and Alzheimer's support strategies. Familiarity with HIPAA regulations to protect resident privacy and confidentiality. Excellent observational skills for patient monitoring and recognizing early health concerns or behavioral changes. Effective cleaning skills to maintain hygienic living spaces in accordance with infection control standards. Knowledge of emergency procedures including CPR certification and first aid training. Compassionate attitude with strong interpersonal skills for working with people with disabilities—including behavioral disabilities Ability to work independently as well as part of a collaborative team dedicated to providing high-quality resident-centered support.
