Job Summary:
Manages all admissions activities for Saluda Nursing & Rehab Center. Promotes the facility's healthcare services, programs, and resources to prospective residents, family members, hospitals, and physician offices, delivering outstanding customer service and cultivating referral networks to achieve occupancy targets.
Duties/Responsibilities:
1. Take inquiry calls from hospitals, families, etc. 2. Ensure responses to inquiries and referrals are handled properly, timely, and with appropriate follow-up. 3. Conducts tours in the facility for possible new admissions. 4. Responsible for verifying and coordinating all aspects of the admission process, including confirmation of payer source, identification of pre-authorization requirements, assessment of social service needs, designation of the Responsible Party (RP), review of advance directives, determination of skilled nursing and therapy care needs, evaluation of medication costs, and development of discharge planning arrangements. 5. Educate residents, families, and the community on the admissions process. 6. Send out new referrals to the Admission Review team and conduct team review meetings to determine acceptance status. 7. Able to clinically review all referrals. 8. Responsible for uploading admission documents into the Electronic Health Records System. 9. Demonstrate a sense of urgency regarding census/premium mix enhancement and management. 10. Responsible for scheduling and completing pre-admission conferences and visiting new admissions of the facility to ensure all needs are being met. 11. Manage hospital referrals on electronic referral management platforms and respond in a timely manner. 12. Assess for Medicaid eligibility and assist with the Medicaid application process through completion. 13. Ensure consistent and effective process for coordination of admissions and bed management with appropriate departments and staff. 14. Coordinates with Maintenance and Housekeeping to ensure inventory and renovation work is completed for reserved occupancy dates for the HRC. Also responsible for notifying Maintenance and Housekeeping when units are vacated. 15. Responsible for communicating admissions/transfers/discharges to appropriate departments via secure care messaging. 16. Call hospitals to follow up with case managers when residents are out at the hospital and send secure care message to staff updating them on the status of the resident. 17. Initiate the bed hold process when a resident goes out to the hospital and pass along the appropriate information to the resident's social worker to call the RP. 18. Initiate the Medicare coverage notice form upon a resident's return from the hospital and pass the information along to the appropriate social worker for completion. 19. Notifying appropriate pharmacy and medical doctors' offices of new admissions. 20. Ensure company processes are followed to ensure a timely clinical and financial approval process. 21. Updating face-sheets with any changes and distributing them to the appropriate employees along with printing resident labels for the units. 22. Contribute to facility's Quality Assurance program and other committees as assigned. 23. Participate in Medicare, pharmacy, and referral meetings. 24. Adheres to all HIPAA guidelines. 25. Performs other related duties as assigned.

Education and Experience:
- Degree in Social Services or in Business with an emphasis in Healthcare, Management, and/or Marketing.
- 1-2 years related health care experience preferred.
Physical Requirements:
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
