The Special Education Work Experience and Transition Coordinator Teacher is responsible for planning, implementing, and coordinating individualized transition services for young adult students with disabilities ages 14-21. The mission of this role is to transition students from traditional high school academics and develop individual pathways to practical independence, employment readiness, community integration, and self-advocacy. This professional acts as an educator, a case manager, and a community liaison, connecting families with adult service agencies and local employers.
Qualifications:
Bachelor's Degree in Special Education - specific coursework in transition is preferred Wisconsin Department of Public Instruction special education certification, number 1810, 1811, 1801 or 1830 in the high school setting Minimum of 5 years of special education teaching experience Wisconsin Teaching License that includes students between the ages of 14-21 Current First Aid/CPR certification preferred but not required Commitment to the Pulaski Board of Education Educational Philosophy Demonstrated strengths in positive human relations, cultural competence, problem-solving, and conflict management Demonstrated ability to: Work collaboratively and effectively with colleagues Follow and support emergency or crisis response procedures Perform all technology-based tasks required of the position Maintain confidentiality in all matters Communicate clearly, professionally, and positively with students, staff, parents, and community members

Position Responsibilities:
Assists with development, coordination, and implementation of transition programming and alignment of the continuum of transition services at the secondary level. Engages in professional learning to maintain current understanding of local, state, and federal transition policies and practices. Ensures special education staff are informed of current transition-related practices, information, and methods for facilitating transition planning that result in positive post-school outcomes. Coordinates and facilitates transition-focused meetings with students and families to review resources, timelines, and next steps; provides ongoing support and oversight throughout the referral process and while services remain active. Coordinates transition activities for college, career, and community readiness Collaborates with a variety of stakeholders, including educators, employers, agencies, and families. Oversee/Implement Indicator 14 - maintain current understanding of state and federal transition requirements. Support district efforts related to Indicator 14 to ensure compliance with state and federal transition requirements; maintain current knowledge of regulations and collaborate with staff to ensure accurate and effective implementation. Develops, writes, and implements IEPs along with progress monitoring data collection to fidelity Serves as Local Education Agency (LEA) representative at IEP meetings as designated by the Director of Student Services/Assistant Director of Student Services. Collaborates with building principals, directors, and other key stakeholders to ensure the continuum of transition services and programming are in compliance with state and federal transition requirements and align with district initiatives Facilitates grant writing activities upon request from the Director of Special Education.
