Arizona Mild/Moderate or Moderate/Severe Disabilities or Special Education ID Certificate OR Arizona Standard Professional Teaching Certificate with a Provisional or Full Mild/Moderate or Moderate/Severe Disabilities PreK-12 endorsement Current Arizona IVP Fingerprint Clearance Card
