Position Summary We are looking for a dedicated and compassionate Licensed Practical Nurse (LPN/LVN) to join our team. In this role, you will provide skilled nursing care to patients in their homes, supporting both their medical and personal needs. Responsibilities Assess patient needs including physical, emotional, and social aspects Provide direct nursing care in accordance with care plans Administer medications as ordered by physicians Educate and support patients and families in managing care needs Monitor, document, and report patient condition and response to treatment Assist in coordinating additional services for comprehensive care Participate in care conferences when needed Maintain accurate and compliant patient records Follow all agency policies and procedures Qualifications Active LPN/LVN license in the state of New Mexico Minimum of 1 year of experience required Strong clinical and patient care skills Ability to work independently and make sound decisions Excellent communication and documentation skills Compassionate and patient-focused approach Why Join Us Work with a team that values the care you provide and the impact you make every day.
