Geriatric Care Manager

Great opportunity for a Social Worker or Nurse who wants to help Seniors and their families age well. This position is both in the field as well as virtual as you will be helping people throughout our company. Successful candidate will have five plus years working with Seniors in the community, have great assessment and communication skills and be able to be proactive. About Us Caring People Inc. is a privately held home health care agency that has been in business for over 20 years comprising of 15 branches throughout New York, New Jersey, Connecticut, Florida and Texas. We are currently looking for a PRN Geriatric Care Manager to join our team who has superb experience working in the geriatric population. The Care Manager will report to the Director of Care Management and is responsible for providing quality professional care management services to all clients and their responsible parties. Our objective is to assist our clients in managing and navigating challenges in aging as well as Adults with physical and mental disabilities and providing the highest quality of life.

This includes:

Care Coordination Medical oversight Interfacing with medical personnel Advocacy, information and referrals

Pay/Benefits:

Minimum of 5+ years experience with the geriatric population in the community Master's Degree highly preferred, but not a requirement Recognizes industry trends and problem solves Self-motivated and work with own initiative, professional and positive approach, commitment to customer service Respectful of company and client confidentiality Personalized and compassionate service with Person-Centered approach Ability to provide non-directive guidance and facilitate constructive relationships Demonstrates Flexibility and Accessibility•This includes outside business hours if needed Quality control•care management services adhere to Aging Life Care Associates Standards of Practice and Pledge of Ethics Strong in building relationships, team player and able to communicate at all levels Ability to provide coordinated communication between family members, doctors and other professionals, and service providers
